• How to fill it out:
  Visit:http://survey.fcu.edu.tw/Login.aspx
  Click “Current Students” → Log in with your NID → Select “Health Self-Assessment Card

Freshmen Health Examination

Please note: This examination is different from health certificate for resident application. The university will coordinate the date and designated hospital for your health check.

On-Campus Examination

Registration Period: July 20 – August 17, 2025
Examination Dates & Times:

• September 4, 5, 6, 8: 08:00–16:00
• September 9: 08:00–12:00

Location: 1st Floor, Recreation Building
Important: Please fast for 6–8 hours before the check-up.
Fee: NTD 575
How to Register:

• Download FCU App
• Go to: MyFCU → Campus Life → Activity → Appointment
• Each student can register once only

Bring along:

• ARC (or passport)
• NHI card (if available)

Off-Campus at Contracted Hospital

If you cannot attend the on-campus check, please visit:

Period: September 10 – October 17, 2025
Registration Time:

• Wednesday: 08:00–11:00
• Friday: 13:30–16:00
  Fasting: 6–8 hours required

Bring:

• ARC or passport
• NHI card (if available)
• Student ID card

Fee (outside regular hours):

• With NHI: NTD 200
• Without NHI: NTD 800

Students may also complete the health check at a nearby hospital.

Steps:

• • Download and use the Feng Chia University Student Health Examination Form only
  • Fee: Approx. NTD 2,000 – 3,500 (varies by hospital)
  • Only forms completed after June 1, 2025 will be accepted
  • Submit the completed form to the Health Center by September 30, 2025
