Freshmen Health Assessment Questionnaire Filling period: February 2 – March 16

• Freshmen Health Examination
  The “Freshmen Health Examination” is different from the “Residence or Settlement Health Examination.” After arriving at the university, students will follow the school’s arrangements to go to the designated location for the examination. Every freshman must participate in the on-campus health examination!

2026 Spring Freshmen Health Examination Instructions

I. On-campus Unified Health Examination

1. 1. Date & Time: March 6, 2026 (Friday), 8:00 AM – 12:00 PM (Check-in closes at 11:40 AM)
   2. Location: Recreation Building (fasting required for 6–8 hours)
   3. Online Registration Period: January 12 – February 20, 2026
   4. Fee: NT$575 (payable in cash or via LinePay)
   5. Registration Path: Feng Chia 3 APP→ Myfcu → Campus Life → General Activities (Freshmen Health Examination)
      (Each student may register for only one session and bring your passport.)
   6. Health Examination Information

 If you are unable to attend the on-campus health examination, please refer to the following options. You must complete one of them.

II. Examination at Contracted Hospital (Tung’s Taichung MetroHarbor Hospital)

1. Available Dates: March 7 – April 10, 2026
2. Check-in Times (fasting required for 6–8 hours):
   Wednesday mornings: 08:00 – 11:00
   Friday afternoons: 13:30 – 16:00
   (If you attend outside these times, in addition to the NT$575 examination fee, an extra NT$200 registration fee will be charged.)
3. Additional Notes:
   (1) Bring your ID card (passport), National Health Insurance card, and student ID to the contracted hospital (Tung’s Taichung MetroHarbor Hospital, Wuqi Branch).
   (2) No appointment required; go directly during check-in hours. Public transportation is recommended.

III. Examination at Nearby Hospitals (Self-arranged)

Please first download the Feng Chia University Student Health Examination Form from the university website and bring it to the hospital for the examination. Fees vary depending on the hospital (NT$2,000–3,500).

2. Other formats of health examination reports are not accepted. If you already have another health examination report (dated after September 1, 2025), you must transcribe the complete data into the Feng Chia University Student Health Examination Form and submit it to the Health Center, FCU.
